She is probably better known for her off screen exploits, including a romantic involvement with Sylvester Stallone, with whom she co-starred in Rocky IV. She starred in the female action hero movie Red Sonja, and she was intended to star as "She-Hulk" in 1989 for New World Pictures, but the movie never occurred, although there are promotional photos that were taken in an attempt to gain financial backing for the movie. Nielsen made headlines again in 2018 when she announced she was pregnant with her fifth child at the age of 54!
