There are two tiny stamps in the upper right blank background. There is 4" of separation at the left and right ends of the middle foldline and a tear at the left end of the separation at right (there is white paper tape on the back of the entire foldline). There is white paper tape on the back of tears and paper loss in the top and bottom borders. There is a long brown stain and two areas of paper loss in the lower left corner. There are tiny tears, creases, and tiny areas of paper loss in the top blank border.
