In this photo you can clearly see that he was having a real weight issue, given his bulging belly, but that was nothing compared to how he would be a decade later. Note that this is a test photo used to show one of the many costumes that Marlon Brando wore during this movie (the stars would put on each costume and stand next to a board with the details of when that costume was used in the movie, and then they would try on a different one and repeat the process, until they had tried on all their costumes; I wonder if major stars of today have to endure this same ritual?).
